Access 2000 form and/or report

I have a "Tracking Form" that we use to route changes around to different people. On the top of the form lists info regarding the changes, including Control Number, issued by, issue date, return by and close date. The second half of this form lists names of the persons that it is being routed to. The user fills in the top portion and then checks the checkbox next to the persons name in the lower half that they want to route it to. I have this set up so that ALL this information is in one table. The problem lies in that I want a second form "Tracking Form Router" that the user can use to fill in after they have done their task on the Tracking Form. When the form opens it has the Control Number and the basic info from the top portion of the Tracking Form, it also lists EVERY person (because it is in the same table) even if the person is not checked to recieve the Tracking Form. They will then fill in the Date Completed and fill in the name of the person they send the Tracking Form to next. I want to be able to have this form ONLY list the persons names from the Tracking Form that have been checked (not ALL of them). I'm looking for a way to normalize it by breaking into more than one table I guess but am not sure how or what I would link together to make it work. Any suggestions?
 
Can you post your table structure? Also, you can use a query to limit records to show only those that are checked. Are you storing the checked names in a table or do you need to read the form itself?
