AAR and H.323 Trunk to Avaya

Have a situation thus. CallManager using MGCP g/w in Europe. Calls to U.S. travel via Frame Relay to an Avaya switch configured as an H.323 Gateway in the CallManager.

USA Avaya switch has 4 digit extensions. Users dial 333.XXXX and 333 is stripped Pre-Dot and the 4-digits are sent to the Avaya H.323 G/W

So, can AAR work? H.323 G/W is in it's own AAR group. IP Phone in Europe is in it's own AAR Group. AAR Group are configured:
Europe to USA: Prefex 001222333 (where 222333 is area code exchange)

Location is set and out-of-bandwidth message is working, however, I am getting busy and the AAR group does not appear to be adding the prefix.

Perhaps I cannot fail over to the PSTN this way since I cannot add the External Mask? (since the devices are all Avaya)
