I have a BCM 4.0 that if the call is answered by the AA and someone dials an extension, the call goes directly to the users voice mailbox. It's all extensions and none of the sets have call forward to the VM DN turned on. Pretty perplexing.. If you dial from an internal extension to another internal extension it rings and then forwards as normal. It's only when someone dials from the auto attendant..
