A write error occurred on the media. (24615)

Hi,
I have been running backups flawlessly for about 8 months. Now I can never get a FULL backup. My Incs' complete fine.
Hardware/Software
- IBM 4560SLX LTO Library, fiver connected to 2109 Brocade.
- Backup server is Netware 6sp3 and BK9{4202}
- QLogic QL2200 HAM Driver
Version 6.50.18 July 11, 2002

I am running into the vendor pointing issue. Veritas states it is the hardware, while IBM states it's Veritas.
I have replaced all LTO drives and used brand new tapes. To me it seems Veritas is not writing the End-of-Media and then it times out after 10 minutes.
Here are some lines from the BK logs.
