How do you add a form to a report?

I created a form with two drop-downs that allow me to select certain criteria, and it has a button that kicks off a report using that criteria.

However, I'd rather that the drop-downs exist in the report itself, rather than having to kick off a separate form.

Can you do that, or is it better to just generate the report from the form?

Have you tried creating a parameter query to build your report from? Whatever info is in the drop down field, you can create a prompt in the criteria field of the query. An exmple would be a drop down called workcenter, this list has a,b,c,d etc.In the criteria I would enter
[Enter workcenter] When the report is opened, I would be prompted to enter a WC, when I enter b, the report would pull all b's. You can also do this with dates, or any other fields from your form.

I like the idea. But two questions:

1. If I have multiple criteria, how can I display them at once? I'd rather have one screen that asks for 3 criteria than a series of 3 questions to answer.

2. With a parameter query, how can I make the response through a drop-down or list box? The parameter queries I've used so far force you to type in a response.
