Usually that is an indication that the connection to the network is not working. it is getting power but no network. I would check all the cables and even bring the phone right to the switch and plug it in.

Network is fine. If you plug another 9630 in, no problem. I've very rarely seen hardware issues. Possible bad firmware load? Is it possible to update the firmware through the USB?
Not that i know of. it sounds like the phone may be bad. are any pins bent in the back of the phone? it may need to be replaced. I havent seen this on any 96xx we have installed.
