9504 key pad stop working?

Hello,

I have an issue with couple of 9504 key pad that stop wotking for no reason. To fix issue customer have to unplug the phone.

IPO was on 9.1.7 and i just upgrade to latest 9.1.11 and still have problem.

Some phone are on the combo card and the other on a 16 digital module.

Any ideas why i have this intermittent problem?
