8 Port Analog Failing

I had an 8 Port analog module fail last week. It is connected to an IP406 with about 45 digital phones and 15 IP phones. I replaced the module with a new one and today the new one stopped working and the light on the front went orange. After a reboot it came back up, but I am getting worried something is going on. We use the analog to send incoming numbers back out to a fax server that routes about 30 different numbers.

Any ideas on what is happening. We are running 2.0.16 on all modules. We have had no problems for over 6 months.
