6416D going off hook while sitting idle

I am doing a list trace station on a set that sits completely idle but the trace shows active then idle within a second, some times the same moment. Avaya has been engaged, but I am waiting for call back.
I think this is leading to problem with customers calling in and then getting dropped when they hit that agent.
The system is a prologix R10 latest load.
So far I have changed TN2402 processor, Changed DS1 board,
Changed set, changed port, changed TN2224cp board, moved set from A cab to B cab.
Has anyone ever seen this happen before?

1. Are there any bridged appearances for this station?
2. Is there an EC500 station assignment for this station?

There are no Bridged appearance keys for that station.
EC500 is not licensed. Any other ideas??
 
Have you replaced the station? Checked the station wiring? Does the station use a headset? Are all of those things in working order?

Station set replaced once and no change.
Cable run to set changed entirely and no change.
The station does not use a headset.

I have been troubleshooting this for months, first thought
it was PRI but that has been checked many times with telco
and it is running clean.
