568B x 12 punch down block?

I need to know the designation/name of the following RJ21X- type punch down block: The right side front is T-R pairs punched in left-right, groups four pairs down, and each four pair group is hardwired to a 568B port. There's 12 of those down the left side of the block, so that's 48 pairs (twelve CAT 5s)it accommodates. I picked up one from a disconnected AT&T system, I think, and kept it until the other day when I found a good use for it. But now I realize how handy the design is, but I need to know what to order--or where. Does anyone recognize this?
 
Certainly recgonize what you are referencing, but haven't seen them in over 10 years. A 12 port patch panel in 66-block format would do the same, just not as convienient as you can't punch it from the front.
 
I believe the correct spelling is: SIEMON Company, and their product part number, S66M2-5T-128LR, looks like what you are describing. See it on page 11.5 in the 2005 catalogue. You might also find it on their website:

being an Avaya tech also, it appears to be a 66 block that was connected to a particular card that wasnt a straight 24 pair card......i would say it was a 4 wire digital card and it most likely was a 16 port card

Skip--The Hubbell piece is the one. Thanks. I didn't know they were CAT 3, though. Thankfully, the app in this case was a set of Merlin drops. The CAT 5E 12 port blocks I suppose are preferable, but since they're 110 wiring from the back, it's not quite as convenient to use, as TTT says.
