5602SW+ IP Phone keeps discovering

I have searched forum extensively but unable to resolve 5602sw+ model phone as follows:

When i connect the phone it initialiazes and uncompresses i02d02p2_887.bin file and loads 46xxupgrade.scr files and executes 46xxsettings.txt via tftp (using MV_IPTel). After that the phone continues in discover mode - it continues to discover the CallSv (Gatekeeper) IP address.

Everything is on the same LAN.
IPO 406V2, s/w ver 4.2.11
Phone MOdel 5602SW+

Is there a way to completely erase the pre-existing binary?
Also, when i use any Mute/Hold commands the phone simply Restarts and enters the above loop...

do you enter the values manually into the phone or are you using DHCP to let it take care of all the settings.

i am entering the values directly/manually thru MUTE-ADDR entry.

actually this site is 2 nos of IPO406V2 both under 4.2.11 and connected via customer's WAN. Apart from that there are 5 other locations countrywide each having one IP phone.

This problem is with one IP phone only..phone itself is fine, it worked earlier.

46xxupgrade.scr files are original.
