5520 Cascade stack broken

We have 2 Nortel 5520 and build to one Cascade stack. They use same FW and SW version. But always broken and no reason. Loop test is fine, no errors on the hardware, Cascade ports, and Cascade link. Any one has any idea about why?

Before Stack broken, we reveived a trap anout stack unit change. "Major: The stack manager has detected a change of status with a stack member". Does any one know FW 4.2.0.13 is good for Cascade or not?

BACKGROUND
After upgrading to 4.2.2.10/11 code some customers have experienced a unit lockup or reset of a
non-base unit in an ERS 55xx stack. In the reported incidents, the problem has only been
exhibited on some but not all stacks in the network. When in the locked up state, a unit will leave
the stack and requires power cycling to return to normal operation and rejoin the stack. The
symptoms of the failure are that the specific locked up has left the stack, it does not respond to
console input and the affected unit is non-base unit. Local switching on the individual unit is still
operational.
