5500 Console BLF tiles appearing multiple times

Hi All,
1 of my guys went out to a site where the customer has added 3 new users to the operator BLF field. When she adds the extensions onto the BLF field ( in tile form) it adds it twice, Up the top in alphabetical order and down at the bottom of the tiles as well. Any suggestions on what might be causing this?
