406 Forbidden - ISA denied the specified url

Symptoms: When clients try to access the Internet via ISA 2000, they get “Troubleshooting 406 Forbidden - Internet Security and Acceleration Server denies the specified url (12202)”

What they can do and did
1. Upgraded latest sp.
2. Tried to Install firewall client and use web client.
3. No errors on Event Viewer.
4. ISA can access the Internet without problem.
5. Clients can ping any webs.
6. We can see the clients on ISA Monitor.

Any suggestions?
