4-pr. to (4x) RJ11 modular CO-line Cable

I'm trying to find a patch cable, which I have seen used primarily on AT&T/Lucent/Avaya system installs to connect CO-lines from a 66-block to the line ports.

It is a bare-end 4-pr. cable split into (4) RJ11 (1-pr.) modular plugs. There is a label with the name "Berk Tek", but no part number on it.

I tried to check Berk Tek's website, searched the web for octopus, splitter & "Y" cables, and checked with my local Graybar, but cannot find this cable. Graybar & others said they could custom make these cables for me, but it must be cheaper to buy them off the shelf. (I would prefer to buy factory made cables rather than make them myself.)

Can any Avaya tech out there tell me the part #, or where to buy these? Thanks!
