I have a BCM 200 on 4.0 with the drive most likely on it's way out. I have a preloaded 4.0 drive ready. Must I do a backup and restore from the old drive to the new drive? If all of my telephony programming is on the MSC card what would be missing? (voicemail, cdr, moh)

As I recall you'll be missing anything to do with voicemail or IP telephony.
Hope you have a recent backup to restore, as a backup from a failing drive would definitely be problematical.

I would do a full backup a couple of times. The backup will contain the licence that you will need to do a restore on.
Make sure that you apply the last release of patches first such as the System and desktop ones before you do the restore.
You will run into problems if you don't do as I've suggested. I found this out the hard way and I had to do another image again.
