3-way conference call issue on BCM400 ver. 4.0.2.03a

I currently have an issue with conference calls on my BCM400 System version 4.0.2.03a. Everytime someone tries to create a 3 way conference call one of the users stays out of the conference even when the IP Phones display that the conference was created succesfully.

I've had this issue in the past and it's corrected everytime I install new patches to the system but a couple of months later it appears again...

You guys have any idea what can be wrong?
