2k3/fax2mail error after install

Running outlook 2k3 and fax2mail app on our workstations. On one pc, after installing, I start outlook 2k3 and get
message -

intelliSEND Add-in Error
Method: AddControls
Source: XPOLX
Error: 5Bh (91)
Desc: Object variable or With block variable not set

Now I have no install problems on any other pc, just this single one. Tried removing/readding, installed as admin., detect/repair, recreated ost.
I can see it in Forms when I open and it is usable from there, its not an option on the New pulldown menu. It is an option on every other pc.

Ive googled till the cows came home...
