2050 3.4 users must click headset button several times

We are seeing 2050 users (3.4) having to click the headset button several time in order to have full speech path.

The option in the phone is set for Off-hook-default-path = Headset.

this is happening on both inbound ACD (CC6) calls as well as outbound calls.

just started after upgrading 60+ from 3.0 & 3.2.
