200icp that will not call fwd always to external cell fon 1

I have replaced a 200el with a 200icp. I cannot get the call forward always feature to work. The feature key on the set lights up, The system abbrev string dialed at the site works but when you call the end user's DID from the outside world you get the embedded vmail box, you get the embedded vmail box when xfered by the receptionist as well.
The sys option 21 is enabled. option 200 in the COS of the set is disabled and option 208 and 238 were already enabled.
What gives?
 
How about options 318 in the trunk COS and 319 in the set COS? Do you have the interconnect table set to allow trunk to trunk access? Is the DID the same number as the extension or are you using Form 55? If neither and the DID is an appearance on the phone then you need to enable option 258 in the set's COS. Are you forwarding to the VM via tenant or call forwarding on the set?

Try making the CFWA destination another extension to see if that works if it does then something is blocking the external forwarding which is normally one of the things I mentioned above.
