We have IPO 500 v2 9.1, phone 1416 reboots on its own, I changed the phone still the same problem.

Bad connection
Cable issue
Floor point / socket issue
port issue.

Try moving to a different port on switch
Change cable between phone and outlet..

Firmware issue
IP address conflict. Ignore this as it is digital

 
Just to clarify, you're definitely talking about the 1416 Digital handset? Not the almost identical 1616 or 1616L IP handset?

I connected phone digital 1416 directly to the port of the card combo module, still the same problem.
 
Did you change the cable from the phone to the port.

Try the phone on another port to rule out the port position.
 
I changed the phone cable same problem when I plugged in another port(combo card) the phone works fine.
 
Sounds like a bad port. Three thing go wrong with phones. Endpoint. Cable. Or port. If directly connected and it is a problem it is the port.
