Is there a way to see the phone firmware version from the phone menu?

It's under the Avaya menu. The button that looks like an A. A/Advanced/self test.

Is there a way to upgrade the firmware on the phone to R55 / 15 directly without going through an IP Office?

no, the firmware is provided by the IP Office the phone is connected to.
if your IPO is old then your firmware on the digital phones is matching and also old.
