BCM 400 4.0 fully patched. Still having issues with attaching KEM to 1140e. Kem's are basically blank and then the set itself locks up. Anyone experiencing the same issues, if so is there a cure yet?

Do you have UTPS 400.203 installed?
Here's a description of the issue fixed:
1. The LCD on the KEM modules going blank intermittently
The following latest firmware is being included in the BCM 4.0 build which
fixes the 1110 series expansion module's issues.
0623C4L.bin - for 1110 (Next Gen) IP Phone
0624C4L.bin - for 1120 (Next Gen) IP Phone
0625C4L.bin - for 1140 (Next Gen) IP Phone

Sounds about right.

Make sure you don't program lines on that KEM because as soon as you assign it under cap/kim assigment the IP phone attached will freeze with that utps-203 patch.

Here is the only issue currently acknowledged by Nortel:

ISSUE: The KEM on an IP set can lose line appearance programming if the BCM is reset, or loses power.
RELEASE: BCM50 R3 and BCM 4.0
CR: Q01730834
SYMPTOMS: Line appearances are missing on the KEM. Line appearances previously on the KEM migrate to IP set.
WORKAROUND: None
STATUS: Investigation.
RECOMMENDATIONS: Please contact Nortel Technical Support (NETS) organization and open a case if you believe you
have a system with any of the symptoms above.
Nortel has released a bulletin to provide status and updates to this issue. Please refer to bulletin 2008008833, entitled:
Business Communication Manager (BCM) - Loss of IP KEM programming on BCM reboot

Thanks guys and gals... I have two hunt groups assigned to the KEMs. I'll move them off the Kems and see if that makes a difference.

Here's the deal. Removing DN's from CAP list in Global settings did the trick. i also removed the hunt group appearances from the kem's. now they are good to go. unfortunately the only way to remove the affected DN's was to enter in another DN on the system. anyone got any better ways to get rid of DN's out of that list?
