1140e expansion module is blank

I have a bcm r 3, the 1140e expansion module keeps going blank. It is happening intermittenly, but on all 3 of the 1140e phones w/ exp mods. These are my only ip phones. I am using the BES50FE 24 port switch. I have the phones into the regular ports and I have an external power supply. I am going to try to switch to the POE ports and not use the external power supply. This happens a few times a day, any ideas?
 
This is a bcm 50 release 3, I have changed out the phone to a POE port. Same thing happens: phone works, then expansion module goes blank. This happens daily.
 
Have you patched the system??

What doesn't kill you only makes you stronger.
 
Patch it up.
A Smart Update for 3.0 was released by Nortel yesterday.

Just a follow up. I worked with a Nortel engineer, who tested in a lab, to duplicate the problem. She found a problem and Nortel created a firmware patch for the 1140e phones. We applied the firmware, and haven't had a problem for about 10 days. Should be good. Thanks for everyone's input.
