Customer was experiencing calls being disconnected when they put the calls from lines 1 & 2 on hold and go to page. We have literally exhausted all troubleshooting avenues including replacing the phone system. This issue continues to happen. We have replaced wiring from demarc to KSU, replaced phones, metered the CO lines and it still continues. We have engaged the phone company in the past with no success. We are engaging them again. Has anyone had this issue or have any suggestions?

Been there, done that. Determined that it follows lines 1 and 2. The issue is as we all know the phone company will declare no trouble and pass it back. We will then kick it back to them because we have done everything we can. I need some info to go back to them with.

What software are we talking about anyway?
I agree, carrier fault but:

Line Polarity reversed?
Supervision from carrier? maybe turn off for test if carrier keeps sending disconnect when not disconnected.
Card replaced?
Software replaced?
NVRAM replaced?
STARTUP performed with no backup restored?

Pretty simple proof after mentioning to them all you have done, from the 4 lines.....move those lines to ports 3 and 4 and vice versa and tell them what you have done and if same issue then you just proved its carrier issue. You did this but did you show them?.
Note that you must also test/show incoming calls to actual lines 3 and 4 to ports 201 and 202, that will prove the first 2 ports are indeed ok as well.

Check meter on all lines (idle and in in use) and note and compare readings too.

No polarity reversal on any wiring that can be seen. The whole KSU, kit and kaboodle was replaced. I have not turned off Supervision. That may be a good test.
