if the vm is turned off on the US switch and the forwarded call continuously rings at remote site the issue is not with the US switch but rather the switch in Canada. Try calling the remote site directly and see if the vm picks up and how long it takes to do so
Did you try deleting the user and then rebuilding it? I recall instances when a user was a part of a hunt group which had a message so it set off the indicator on the phone and even though you turned off vm for that group their is no indicator to turn off vm light on particular phone.
upon installing phone manager everything appears to install with no issues but when attempting to open actual program "invalid stream format" prompt appears and program does not open
