I've used this script on lots of other sites, and never got this problem before.
The HTML form calls for posting to mydomain.com/cgi-bin/contactForm.pl with a recipient value of 1.
Here's the top part of the form

The tags you show are character-level tags--meaning the affect what's contained within the beginning/ending set. Start with <i>some text<i> end with it and those words are italic.
Paragraph level tags begin as such
@stylesheetname:Text and so on

jimoblak makes a good point - if these are the "template" variety tear-outs, measure the areas for one and step/repeat for the offset dimensions on the physical sheet - OR and you have MS Word, they have Avery and some other brand set-up guides - write down the measurements and draw the guides...
Make sure you're using the right tag format - no spaces, tabs, etc. - beginning of the paragraphs (@stylesheetname:Text to be used in the paragraph...) and that the file is save as ascii text with the .txt extension. Other than that, give a small sampling and we'll look.
