What version are you using. In 2006, using Etransmit has made such a task a complete breeze. If you are using previous versions it becomes incresingly more difficult.
Assuming you have at least 2002, use eTransmit. Here are the steps

I noticed that the mtextfixed is a newer system variable thatn 2000. I should have recognized that from the begining.

-When going to settings on the qleader, make sure that under Annotation reuse, that "None" is selected and as you have stated already that under Annotation that Mtext is sdlected.
-Qtextmode should still be 0, by default.

Just a quick reiteration on Psltscale
0 = Scaled by the global LTSCALE factor. (half the Dimscale)
1 = Viewport scaling governs linetype scaling, whatever it is set to. Highlight viewport and click properties. Use regen an you may see difference.
Well here is my best explanation.
In your base drawing (where your work is) the Ltscale should be half the Dimscale. So that means, if the floor plan, or in your case the site/ civil plan is 1"=30'-0", the Dimscale would be 360 and the Ltscale should be half, or 180.
