Did do this once - think I just put all the column headings into the 'Header' page - so went into layout editor, as normal, cut the field/column headings there, then switched to the header page and pasted them there... think that worked fine...?
there's an SRW package "Run_Report" which you could put in an appropriate trigger - possibly an AfterReport Trigger?
