Unable to make outbound calls without dialing twice
This might be a long shot but did you phone provider turn the other two outward facing DID's to centrex lines? Try dialing 9 then 9 again then the number. If that works call your provider.- Dariusz2k

Redial on 2420's
On the extension that's having the issue when you plug in the other 2420 that has the working redial into it and hit redial does it work? if it does the 2420 might just have a faulty redial button. If it doesn't though check the extension's settings in ASA and check to make sure you have it set...- Dariusz2k

3rd party voice mail that will integrate with Avaya
We've looked at AVST,Microsoft and Aura(Which is the Avaya Solution) the lowest cost solution was Avaya's Aura by far. Licenses are the biggest cost of the system overall but it's end of year so you could probably get discounts on all sorts of Avaya stuff.- Dariusz2k
