acrobat reader does support hyperlinks. But you need to create hyperlinks using the link tool available in the full version Acrobat. when u convert a word document into a pdf document using acrobat distller the links do get lost, i am not sure what happens when you use a third party software...
Before making the fields using the form tool, go to the tools section in Acrobat select forms, select fields and check set tab order, you will see an arrow and a hash sign. Then you go about making the fields in the order you want users to fill it in.
