We are working to test with a POTS phone when the tech is on site this week. The device is a telephone line remote load on hold player manufactured by Premier Technologies in MN. We have replaced the player with a known good one, no change in the operation.- CowboyinCognito

Mitel 3300 4.1 We have a device on an analog port that we call into using a DID number. When we dial the device from an outside POTS line, the device receives DTMF tones from us, but we can not any hear DTMF tones generated by the device.
