1. Open the Document Explorer: menu View -> Drawing Explorer Window
2. The top item in the window is the drawing name, right click and menu Show ShapeSheet. This will bring up the document shapesheet.
3. Insert -> Section: User-defined cells
and create a row containing the name of the document...

2. Insert it into your diagram using Insert -> Image and resize and locate the image to suit.
3. With this image selected, menu Insert -> Hyperlink
and browse to your spreadsheet.

It could be that Visio isn't the ideal tool for this, use your favourite search engine to search for "Create a polar diagram". Any diagram you create can be imported into Visio though.

DO NOT UNGROUP SHAPES!
There is information stored within the group shape that is lost as soon as you do the ungroup.
There are other ways to edit grouped shapes.
