Here's the weird question that the article MS05-019 doesn't answer: It says the hotfix can be used on Windows 2003 SP1--but what if you have SP2 on the server? Was the "issue" corrected when SP2 came out, or just carried through?

I'm assuming you've done the adprep and forestprep before putting up the Exchange 2003 server. I believe you need to set the routing group member/master roles, as appropriate. Check that under the Routing Group>Members folder in the tree.

Just to make sure--both Exchange servers are in the same organization/admin group, correct?
I'd try probably enabling the anonymous client permission to contributor.
One other thing to try--try adding a replica to the public folder store on the second Exchange server.
