503 service unavailable error in web page and the CCMA_DefaultAppPool service in IIS Application pool is stopped when attempting to access CCMA web page

I have a sip trunk configured with a sip provider in IP Office v2 R11.1 behind a Meraki firewall
No signal issues for incoming and outgoing calls, but no audio in both directions when answering call, and system status shows 100% packet loss with media IP even though there is a ping from IPO

The full avatar appears as it should in the ringtone notification on vantage
But it doesn't look as full as it should after answering and while talking

IP office automatically dials the last number from contact/ call log when user lift the headset /press the speaker button before choosing any number
