Crystal W2 Form

Can I Scan a printed form into Crystal Report?
I am looking for a product or a procedure that would allow me to scan in a printed form and associate fields in the form to fields in the database to create a Crystal Report. Is this possible or am I dreaming? Gary- glmarsh
