Aha! Solved the header and footer problem. It's a setting in the browser itself. I'm using IE 6. Under File/Page Setup, there is a field for header and another for footer. Mine has these values in it:
Header: &w&bPage &p of &P
which puts the document title and Page 1 of 1.

I have a file in Indesign that is letterhead. It has a logo and text at the top of an 8.5" x 11" document.
I saved it as an EPS and opened it in Illustrator. Instead of showing me my logo/text at the top...
By the way, someone else suggested I make the rectangle be the background image for the same div as the fields instead of a separate div. Sounds good on paper, but when I tried that, I still couldn't click on the fields in IE.
I'm definitely open to suggestions, which is why I posted to start with. Anybody know a different way to do this that will still give me the visually effect I'm looking for?
