    If I understand the question (#2), I whould do the following; create an index on the patient database, the key being the patient #.     I am not sure what 'buffermode override 5' is, & what your problem is, but before you pack the table you have to use it exclusively (use xyz excl) and it shouldnt be open by any other user.
