I've read pretty deeply on this but was worried I was missing the obvious. I love the concept of cascading master pages, but applying master pages is much slower in InD than Q if this manual process I'm following is the only way to go.

Yes, under file/document setup, you can change it. However, you may first want to see that you've got the option for "layout adjustment" in your master pages so they'll resize.
Does anyone know why printing from InDesign is so much slower (and PostScript files so much larger) than in Quark. Even pages with just text files is slow. Perhaps it downloads fonts with every page?
