Its going to be hard to diagnose without seeing the config file, or at a minimum a screenshot of the button programming screen for the extn you're having issues with.
This is going to be impossible to diagnose with the information above without potentially seeing your config file from Manager, and knowing exactly what you changed, etc.
'BLFs flashing red' - could indicate that the extn is off hook, solid red could mean that extn has DND activated...
When you say 46xxsettings, is the system not using the auto generated one? Most of the time any customisations would be made to 46xxspecials. Is there a 46xxspecials file that you've missed?

Just an update on this for those interested. Stopped the SIP provider from re-registering every 30 seconds and now registration occurs every 3 minutes (setting in IPO is 6 minutes). It has definitely slowed down the SD card from increasing in size, however hasn't stopped the problem all...
