    Try POWER OFF (not just restart) BOTH Cabinets and try again. This is a known issue with early OS7030 2 cabinet configurations. Was fixed with a CPLD upgrade and software upgrade.

    I've had many a OS7030 KSU loose the onboard SLT ports. Never had a 4SM card go, but only installed a handfull of these cards at most.     Samsung OS7030

    The OS7030 single cabinet can have a maximum of 18 IP Phones connected. The OS7030 Dual cabinet can have a maximum of 16 IP Phones connected. Every 4DM/4SM card reduces the number of IP Phones by 4 for each card (even if there is no handset connected to the port)

    You can have as many mailboxes on a single as you want. It's not like the SVMi4 where if you had a 2 port VM you could only have a certain amount of mailboxes

    Your version of JAVA is old, i would look at updating it to the latest V7 (if you can)
