Check the SQL server where the Datastore resides. Make sure that the DB is still there and make sure you have rights to the DataStore DB.

You shouldn't need to worry about this at all if you are publishing out an application properly.

It sounds like you are publishing the entire desktop.
If so, you can install the Program Neighborood agent on your Citrix servers.
That way when the user logs in the PN Agent will look at their A/D group and put the publish app icon on their desktop.

That is correct. The Citrix server will be the one contacting the License server for the TSCAL.
I would install the license server and TSCAL on the "B" domain.

If you want a license server in your cold site you will simply need to have a server named EXACTLY the same as your production server and copy the lic files over. That should do it.
The ONLY catch is that the name be identical (it is case sensative). I've actually copied my lic file...
