Why am I getting this inetinfo.exe error?

If you are using an old NT server and are receiving the following inetinfo.exe error
Code:
exception access violation (0xc0000005) at address 0x6bc16296
check if:
- This version of w3svc shows that Hotfix MS04-021 was installed.

- Because there is a known issue with Microsoft Windows NT 4.0 server. It is caused when an ISAPI filter registers for the SF_NOTIFY_PREPROC_HEADERS notification and handles the request from this notification. In this case, IIS tries to access a non-existent metadata object, and an access violation occurs.

The following filters have been found to cause this problem: ò Microsoft Proxy 2.0 Web Proxy filter
ò Entrust getAccess filter
ò Netegrity SiteMinder filter

Then: you will need to contact Microsoft and get Hotfix 873401
