How do I format my web pages for printing, independent of the type of printer and page setup of the browser?

No matter how you format your HTML page, the print output shall vary depending in the browser in use, the Page Setup of the browser (headings, footers, margins), and the type of printer & paper size in use.
At present, the only solution I can think of is to create PDF version of the pages for printing. A server-side scripting like ASP is needed to do this on-the-fly (and you may have to buy a PDF software). I've already tried this, and it works.
