One of the simplest mistakes made by beginners, is to build a form with fields placed outside of the <Form> </Form> tags.

So to avoid this, begin your form by using the following method.

Insert >> Form >> Form

This will produce the form surround (the dashed or dotted lines) and both the Submit and Reset buttons.

Next, hit Enter a time or two to create some working room before adding your form fields.

You may also wish to enclose your form inside a borderless table and or set a 2 column table inside this to hold your form fields and labels. Either or both of these will give your form a neater look and allow your form to be positioned using table properties.
