Upgrading BayStack 450-24T software

Hi all,
I am about to upgrade software on pure stack of 3 BayStack 450-24T switches running FW:1.48, SW:4.1.0.6 and ISVN:2. It is the first time for me. I have already downloaded software 4.5.5.03 (b450_4553.img) and checked software release notes. Here are some questions i really worry about,
1. Is it possible to upgrade directly from 4.1.0.6 to 4.5.5.03?
2. Do i need to upgrade software only once for entire stack or upgrade software on each component within a stack?
3. What is the difference between agent code image and diagnostic/boot code? Do i really need to install them both?

You can upgrade directly from 4.1 to 4.5. The upgrade can go against the entire stack at once. Agent code is the software/runtime code. Diagnostics is just that - diagnostics/low level firmware.

I haven't looked at a 450 in at least 2 years. Typically I update the diags first, then the agent code. The release notes will dictate if they should be upgraded in a different order, or if updating the diag code is even required.
 
Software release comes with b450_4553.img and b450_148_diag.bin. But it seems to me that there is no need to install b450_148_diag.bin as current firmware is already 1.48. I mean is it enough to look at FW:1.48, SW:4.1.0.6, ISVN:2 information to identify what do i really need to install?
 
The diag code doesn't always change between agent code releases. As you'd imagine feature changes and bug fixes happen a lot more often in agent code, esp in older hardware.
 
What happens if software upgrade is suddenly interrupted or failed by some reasons. Do i need to perform some recovery procedures? or switch will simply boot from previous software version?
 
Depends on where it is in the process, if its just in the 'download code' section I think you'd be OK. If its overwriting the flash with the new image you'd be in trouble, although there are ways to still upgrade the agent code from the diag boot menu. Newer switches have enough flash to store two versions of code (old and new) but (as I recall) these older switches could only store one at a time.
