SoundStructure C12 with VoIP Card Disconnecting Calls

Customer has an IP500v2 running 6.0 (8).
Installed a Polycom SoundStructure C12 with VoIP card that connects as a 3rd Party Endpoint to the system (license installed and working).

I was able to get everything working properly; however, on every call, anywhere from 15 seconds to 2 minutes in, the call disconnects.

In IPO, the call just shows as if the person hung up; but in the Polycom logs it shows that the call was dropped.

We already replaced the VoIP card and it did not resolve the issue.
