Does anyone know of a site or have an example of a general "project requirements" document? I am looking for something very general that can be used or altered for most projects.

Also if you know of any where I can get a general software consulting contract to work document.

The generic term for these sorts of things (in general) is "project management templates"; of course you'll find a whole range of templates.
 
Do a Google on PRINCE2 standards. Although PRINCE (Projects in a Controlled Environment) doesn't actually specify what your documents must contain it will tell you what documentation is expected.

I'm guessing that a lot of the sites Google returns will also give example templates for each of the documents you will require.
