php mailform with validation 1

Hi,
Can anyone recommend a good flash 5 tutorial for setting up a mailform that uses php? I also need to validate the form and have it redirect to a thank you. Ideally, I would also like visitors to be able to check an order checkbox. If they do so, I would like their address to be required too.
