IOS crash

What would cause a repeated IOS crash - even if it's an IOS that we've always used? This IOS has been reloaded and configured numerous times, starts to flake out a little by going into <BOOT> mode and back to normal again. Eventually there's that final software crash where nothing works and the reload/reconfig has to happen again.

Could this be a hardware problem?
 
Bad RAM, Bad FLASH, bad load of software, memory leakage when a new feature is enabled .. NAT is known for this. Hitting a critical level on something like the numbers of routes etc.
